Send us the evidence and other documents we ask for within three months from the date that we ask for it. We typically need the following documents (but these may not be all):
If we ask for an original certified copy of a document, we will not accept a certified copy of a previously certified copy. All information must be provided in the format we prescribe and without expense to us.
If the steps above are not followed, and/or the claimant does not send us the information we ask for within the time period given above, you may lose your right to claim. You can send them to us in three ways:
